| SUBMITTING THIS FORM, the undersigned hereby certifies that he/she is acknowledges they have read and agree to 24/7 Express Logistics, Inc Terms and Conditions of Service and Accessorial Charges and the above information is true and correct to the best of his/her knowledge. The applicant also agrees 1) that invoices are net and due upon receipt or established terms after credit approval 2) there are no discounts for early payment 3) that invoices not paid per terms, late fees will be applied as follows: 3a) 10% of total invoice are applied at 30 days. 3b) 15% of total invoice are applied at 45 days. 3c) 20% of total invoice after 60 days 3d) all late fees are accruing 4) to pay all costs related to the collection of any overdue account, including but not limited to, court costs, attorney's fees, and related expenses. 5) to authorize 24/7 Express Logistics, Inc to investigate the applicant's credit record. |

Individual Personal Guarantee |
| (Must be acknowledged and agreed to by Principals) In further consideration and in order to provide further security to 24/7 Express Logistics that Debtor will comply with the terms and conditions of this Agreement, the undersigned personally and unconditionally guarantees that Debtor will make timely payment of all amounts agreed to, due and owing under the terms of the Agreement. If Debtor defaults on its obligation under the Agreement, the undersigned specificaclly acknowledges that he or she will become personally liable for the default and, therefore, 24/7 Express Logistics is not required to seek payment from Debtor or any other source before demanding full payment of unpaid amounts under the Agreement as well as its ability to recover reasonable attorney fees and applicable court costs from the undersigned. The undersigned further acknowledges and agrees that 24/7 Express Logistics may seek to collect on the default by pursuing claims and remedies against the undersigned as well as by continuing its collection/enforcement efforts against Debtor. |
